Rental Contract

• Please complete the Rental Contract if you are booking more than 6 months in advance. If it is your first time renting the theatre, you will be required to meet with the Theatre Director prior to submitting your request. This request must be received with a non-refundable rental deposit AND approved by the Theatre Director to officially reserve a date.

• Once your reservation request is approved, you will need to submit the Rental Contract with as many details of your event as you can provide in advance (ie. dates and times you need access to the theatre, hours for technicians, amenities needed, etc.)

• Please complete the Box Office Agreement Form if you plan to use the Paramount Theatre Box Office Service for selling tickets to your event. If you DO NOT plan to use the Paramount Theatre Box Office Service, you must complete the Box Office Waiver Form.

Effective January 1, 2014, N.C. Gen. Stat. § 105-164.4(a)(10) imposes a privilege tax on the retailer’s net taxable sales or gross receipts to
admission charges to an entertainment activity. All advertised ticket prices for events sold through the Paramount Theatre Box Office
Service includes 6.75% Admissions Tax when applicable. If you do not use the Paramount Theatre Box Office Service, you are
responsible for collecting and reporting these taxes.

Paramount Theatre Supplement Guide

This document contains important information such as details on Fire Watch, Alcoholic Beverages, Film Showings, Poster/Graphics sizes for display in the Paramount lobby, Basic Advertising Contacts, and specifications of the theatre such as stage size, seating charts, and lights/sound equipment.

E-Verify Affidavit Form and IRS Form W-9

These forms are required for first time users of the Paramount Theatre Box Office Service or when changing the individual or organization that is receiving the payment for ticket sales. Both forms must be completed, signed, and the Affidavit form must be notarized.
